On my first visit to the US, a lot of years ago, I was confronted by the
obligatory continent-sized salad (water with the skin on - yuk), so I
left most of it, and was wondering why I was waiting forever for my
steak to arrive. So US restaurants operated the same policy as my
mother - if you don't eat your greens, you don't get anything else :-)
