This is usually an unfortunate consequence of retrofitting a design with 
features not originally planned at design time. The designer isn't to 
blame, and neither is anyone else. You can't predict everything. I've been 
in that unenviable position with sites I've managed - I design for three 
ads to be on the page and 3 months later, management wants another 3. :(
